Objective To study the expressions of osteopontin (OPN),P53,VEGF in the HBV related HCC tissue specimens and the relationships between the OPN expression and the early recurrence and prognosis of the HCC after radical resection. Methods The 237 HBV related HCC specimens originated from the radical hepatectomy patients. The expressions of OPN,P53,VEGF in the 237 HBV related HCC tissues were examined by tissue microarray and two-step immunohistochemistry methods of PV-6000 and the expressions of OPN mRNA were examined in the frozen HCC specimens by the RT-PCR . Results All patients were followed up , the following up time were 1.3~102 months, the median time were 43 months. The 55 patients after radical hepatectomy recurred in the early time , of the total hepatic recurrence rate was 56 %(31/55)and the extrahepatic metastasis rate was 44%(24/55) , but 89 patients recurred in the long term, of the total hepatic recurrence rate was 79.8%( 71/89 )and the extrahepatic metastasis rate was20.2 %( 18 /89 ). The positive expression of OPN in the HCC tissues, paracacinoma tissues and liver cirrhosis tissues were 37%(88/237),17%(14/81)和14%(11/79),It have the statistical significance by compared the expression of the HCC tissue with the expression of the paracacinoma tissues and liver cirrhosis tissues(χ2= 12.44, 16.53,both P<0.001).The positive expression rate of OPN mRNA was 37.5% (15/40). The OPN positive rate is 51%(28/55)in the early recurrence group, 33.7 %(30/89)in the early no recurrence group and 30.9%(29/93)in the no recurrence group, It have the statistical significance by compared the expression of the short time recurrence group with the non-short time recurrence group and the no recurrence group t(χ2= 4.181, 5.679,both P<0.05). The P53 positive rate in the HCC tissue is 25.7%(61/237), the VEGF is 77.2%(183/237), the positive expression rate of P53 and VEGF in the OPN positive group is higher than the OPN negative group, both have the statistical significance(χ2= 4.181、5.679,both P<0.05)。The The positive expression rate of OPN decreased in the listed below patients: the serum AFP≥400μg/L, satellite focis, vessel tumor embolism , histological median and low classification, Ⅲ-Ⅳstage in the TNM staging(all P<0.05). The tumor early recurrence rate(31.8%, 28/88) in the positive expression of OPN group was higher than the negative group(18.1%, 27/149)(χ2=5.624, P=0.016);The median survival time in the positive expression of OPN group was 49.6 months and was shorter than the negative group(χ2= 5.916,P=0.015). The positive expression of OPN was the independent facter for the HCC early recurrence and the overall survival time. Conclusions The positive expression of P53 and VEGF is higher in the positive expression group of OPN, and the positive expression OPN in HCC tissues implys the higher extrahepatic metastases rate, the early recurrence and poor prognosis after radical radical hepatectomy in the HBV related HCC patients .
